Why it matters

The researchers systematically searched MEDLINE, Scopus and Web of Science for eligible randomised controlled trials up to December 2021. Eight trials with a combined 549 participants with metabolic syndrome met the criteria. The pooled outcomes covered glucose control (fasting glucose, insulin and HbA1c), the full lipid profile (triglycerides, total cholesterol, LDL and HDL cholesterol), the inflammation marker hs-CRP, body weight, body mass index, waist circumference and blood pressure. A random-effects model produced weighted mean differences, a dose-response analysis was run, and the certainty of the evidence was assessed with the GRADE system.
What they found
Walnut consumption significantly reduced serum triglycerides, by 0.1 mmol/L on the pooled estimate, which is a real but modest change. Nothing else shifted: total, LDL and HDL cholesterol, fasting glucose, insulin, HbA1c, hs-CRP, body weight, body mass index, waist circumference and both systolic and diastolic blood pressure were all unaffected. The dose-response analysis did detect a significant association between the amount of walnuts eaten and both fasting glucose and HDL cholesterol, hinting that dose may matter even where the overall pooled effect was null.
Where it fits
Nuts in general, and walnuts in particular, are often promoted as broadly cardioprotective, so a result this narrow is a useful correction for this specific group. It does not contradict evidence in other populations; it shows that in adults who already have metabolic syndrome, the measurable trial benefit so far comes down to triglycerides. The authors call for larger and better-designed trials to clarify whether other cardiometabolic benefits exist, and the dose-response signals for fasting glucose and HDL are an obvious place for future trials to look.

If you have metabolic syndrome or its features, this is a reason to see walnuts as a sensible food rather than a lever that will move your weight, blood pressure or blood sugar. The one number the pooled trials show them nudging is triglycerides, and only modestly.
