Fitness trackers alone did not budge blood pressure

Why it matters
Hypertension affects more than 4 billion people worldwide and is a major risk factor for cardiovascular disease, and getting people to move more is one of the most reliable ways to manage it. Wearable devices that count steps and nudge activity have become enormously popular, and it is tempting to assume that buying one translates into measurable health gains. Yet in many countries physical activity levels remain inadequate despite the technology. This review asked the direct question: do interventions built around wearables actually improve blood pressure?
What they did
The authors conducted a systematic review and meta-analysis following PRISMA 2020 guidelines, registered in advance, searching PubMed, MEDLINE, the Cochrane Library and a Japanese database. They identified 21 randomised controlled trials in adults with hypertension, type 2 diabetes or overweight and obesity. The interventions used pedometers, activity trackers and smartwatches, ran from 12 to 48 weeks, and were pooled with random-effects methods. Blood pressure was the main outcome, with body weight, fasting blood glucose and HbA1c examined as secondary outcomes.
What they found
The pooled analysis showed no statistically significant improvement in systolic or diastolic blood pressure at any intervention duration examined: 12, 24 or 48 weeks. The secondary outcomes told the same story, with no significant improvements in body weight, fasting blood glucose or HbA1c. How much the individual trials agreed with each other varied from low to high depending on the outcome and the length of the intervention. The authors conclude the evidence for wearables as a standalone intervention is currently insufficient to establish clinically meaningful improvements in blood pressure or related metabolic measures.
Where it fits
This result complicates the optimistic narrative around consumer health technology. Physical activity itself remains an effective way to manage hypertension; what this analysis questions is whether a device that monitors and prompts activity is enough on its own to deliver that benefit. The authors suggest future work should embed wearables in comprehensive care models combining targeted feedback, education and behavioural support, rather than relying on monitoring alone. Whether such combined approaches succeed remains an open question this review could not answer.
What it means for you
If you own a tracker, this is a reason to see it as a source of information rather than a treatment: across 21 trials, the device alone did not significantly shift blood pressure, weight or blood sugar in people who stood to benefit most. That does not mean the activity itself is worthless, only that strapping on a monitor did not reliably produce enough behaviour change to move these numbers. Anyone counting on a wearable to manage their blood pressure has grounds to think the device needs to be part of a bigger plan.
