Why it matters

Dietary patterns rich in polyphenols are of growing interest for managing cardiometabolic risk, and pigmented rice varieties contain phenolic acids and flavonoids that give them antioxidant properties. Rice is a staple food for a large share of the world, so if simply choosing a coloured variety over a white one improved blood sugar, blood pressure or weight, the public health implications would be considerable. Individual trials have tested this idea in scattered ways, with different rice forms, durations and participants, which is why a structured review of the evidence was needed to see what actually holds up.

The authors systematically searched four electronic databases, PubMed, ProQuest, EBSCOhost and Google Scholar, for relevant English-language articles published since 2000, following PRISMA guidelines with the review registered in advance. Two-stage screening produced seventeen randomised controlled trials, seven testing acute intake and ten testing chronic consumption. Outcomes covered antioxidant status, lipid profile, glucose and insulin, blood pressure and weight in adults. A random-effects model pooled any cardiometabolic outcome that at least three studies had reported.

What they found

Acute intake of pigmented rice increased plasma antioxidant activity and lowered post-meal glucose and insulin levels. With chronic consumption, the pooled results showed fasting glucose lower by 1.60 mg/dL across 5 trials and 349 people, body weight lower by 0.23 kg across 3 trials and 182 people, and diastolic blood pressure lower by 1.39 mmHg across 3 trials and 185 people. Every one of these effects is small. No effect was found on total cholesterol, LDL, HDL, triglycerides, body mass index or systolic blood pressure.

Where it fits

The findings sit comfortably within the broader literature on polyphenol-rich foods: plausible mechanisms, some measurable short-term effects, and modest longer-term signals that need better trials to confirm. The authors themselves note that the small number of studies and the differences between them, in participants' health status, the form of rice used and the duration of the intervention, mean more high-quality trials are needed. Whether the small glucose and blood pressure changes would persist, grow or matter clinically over years of consumption remains an open question.

What it means for you

If you already eat rice regularly, this is a reason to think a pigmented variety is at worst equivalent and possibly a slightly better choice for blood sugar and blood pressure, though the measured differences were tiny. It is not a reason to treat coloured rice as a therapy or to expect any effect on cholesterol, since the lipid measures did not move at all. The honest reading is a food swap with a plausible small upside, resting on a thin evidence base that the reviewers themselves want strengthened.
