Individual trials have reported cognitive improvements, which keeps the story alive in headlines. What has been missing is a systematic pooling of all the randomised evidence to see whether the effect survives when the studies are considered together rather than one at a time. This review and meta-analysis set out to deliver exactly that verdict.
What they did
The authors systematically reviewed randomised controlled trials testing probiotic, prebiotic or fermented food interventions against cognitive performance. Twenty-two studies covering 1551 participants were included: 11 probiotic studies with 724 people, 5 prebiotic studies with 355 people and 6 fermented food studies with 472 people. Results were pooled for global cognition and for individual cognitive domains, and each study's risk of bias was assessed, with 16 of the 22 rated as low risk.

Despite 14 of the 22 individual studies reporting significant improvements in specific cognitive domains, the pooled analysis found no significant effect of any intervention on global cognition. Probiotics, prebiotics and fermented foods all fell short, and no individual cognitive domain showed a significant benefit either. Where it fits
This result complicates a popular narrative. Mechanistic work on the gut-brain axis is intriguing, and scattered positive trials had suggested human cognition might respond to gut-targeted interventions, but the pooled randomised evidence does not currently support that claim. The authors stop short of closing the door: the small, short and varied trials mean a modest effect in specific groups cannot be excluded, and they call for adequately powered studies in clinical populations. For now, the null stands as the best available answer.
What it means for you
If you take a probiotic hoping for better memory, focus or mental sharpness, this is the most comprehensive evidence to date and it found nothing. That is genuinely useful: it is permission to stop expecting cognitive returns from gut supplements, and a reason to treat single-study headlines about fermented foods and brainpower with scepticism. If you enjoy yoghurt, kefir or kimchi for other reasons, nothing here argues against them; the point is simply that a brain boost is not something the pooled trials can find.
