Why it matters

As lifespans lengthen, age-related neurological conditions become more common, and they remain difficult to treat once established. That has driven intense interest in the gut-brain axis, the idea that signals between gut microbes and the central nervous system might be modifiable through diet or supplements. Probiotics are the most commercially visible expression of that idea, and they are widely marketed with cognitive claims attached. This meta-analysis set out to determine the clinical potential of probiotics specifically for improving cognitive functioning in older adults.

What they did

Systematic searches were run in PubMed, Scopus and Web of Science for published randomised controlled trials. Records had to test probiotic supplementation with cognitive functioning as the main outcome; 56 records were identified and screened, and 20 randomised controlled trials met the criteria for analysis. Reported means and standard deviations of cognitive test scores were converted into standardised mean differences and pooled with a random-effects model. Where trials reported them, blood concentrations of pro-inflammatory cytokines were analysed as a secondary outcome.

What they found

Overall, probiotic supplementation tended towards better cognitive performance, with a standardised mean difference of 0.19 and a 95% confidence interval of -0.13 to 0.52 — a small effect whose interval crosses zero. Inflammation moved in the expected direction too, with a standardised mean difference of -0.44 and a confidence interval of -0.94 to 0.06, again not statistically significant. So neither the primary cognitive outcome nor the secondary inflammatory outcome reached significance. The authors emphasise that the set of trials was characterised by high heterogeneity, which prevented determination of a true overall effect size.

Where it fits

The gut-brain axis remains a plausible target, and the direction of both pooled estimates is at least consistent with the hypothesis. But plausibility is not proof, and this synthesis shows the human trial evidence in older adults is not yet strong enough to support confident claims. Heterogeneity is the core problem: the trials differ in strains, doses, durations and the cognitive tests used, so pooling them produces a wide, uninformative interval. Larger and more standardised trials, using consistent cognitive measures, are what would settle the question.

What it means for you

This is a reason to be sceptical of probiotic products sold on brain-health promises for older adults, at least for now. The pooled evidence points gently in a favourable direction without ever crossing the line into a statistically reliable effect. That is not the same as showing probiotics do nothing — it means the trials so far are too varied to tell. Anyone weighing up a purchase can read this as an unresolved question rather than an established benefit.
