Why it matters

Cognitive decline is among the biggest concerns of later life, and exercise is one of the few accessible tools with a plausible protective role. Most of the research attention has gone to aerobic exercise, leaving a live question over whether resistance training, the kind best known for preserving muscle and bone, also supports thinking. A second question matters just as much: whether any benefit survives once cognitive impairment has already set in, or whether by then the window has closed. This review pooled the intervention studies to answer both.

What they did

The authors searched MEDLINE, Embase, CINAHL, SPORTDiscus and AgeLine from inception up to August 2020, with no restriction on publication year, for experimental studies of resistance training and cognitive function in adults aged 60 and over. Eighteen studies made the final analysis: ten in cognitively healthy community-dwelling older adults, seven in cognitively impaired older adults, and one covering both groups. Results were pooled separately for the two populations, with overall cognitive function as the headline outcome and short-term memory, assessed by the digit span test of the WAIS III, examined alongside it.

What they found

Resistance training significantly improved overall cognitive function in both groups, with a pooled effect of 0.54 in cognitively healthy adults and 0.60 in cognitively impaired adults. The picture diverged on short-term memory: a significant improvement appeared only in the cognitively healthy group, not in those already impaired. In short, lifting moved global cognition regardless of starting status, but the memory-specific benefit did not extend to everyone.

Where it fits

Most of the exercise and brain-health literature has centred on aerobic training, so pooled evidence that resistance training improves overall cognitive function widens the case that more than one kind of exercise is brain-relevant. The similar-sized effect in cognitively impaired adults suggests impairment does not close the door on benefit, at least for overall function. The divergence on short-term memory hints that specific cognitive domains may respond differently once impairment sets in. Why that is, and which training dose or format matters most, remain open questions this pooled analysis cannot settle.

What it means for you

If you think of lifting as purely a muscle and bone pursuit, this is evidence to widen the frame: in pooled studies of adults aged 60 and over, resistance training moved overall cognitive function in the right direction. That held whether cognition was healthy or already impaired, which is arguably the more encouraging half of the finding. The usual cautions apply, since the underlying studies varied and the precise size of the benefit is uncertain, but the direction of the evidence favours the weight room being good for the head as well as the body.
