    A systematic review and meta-analysis of 35 randomised trials in adults aged 50 and over links combined lifestyle measures, Mediterranean-style diet, aerobic and resistance exercise, cognitive engagement, social support, sleep and preventive care, with better ageing outcomes, including a 25% lower risk of cognitive decline with 7-8 hours of sleep.

    Walking-based rehab gets older adults moving again after illness

    In a meta-analysis of 29 randomised trials with 7,076 older adults, community rehabilitation built around outdoor walking plus behaviour-change support increased physical activity, endurance and outdoor mobility in those recovering from illness or injury, with effects preserved at follow-up where reported. Falls-related self-efficacy did not improve.

    What it shows: Meta-analysis of 29 RCTs (7,076 participants), but 66% of trials were at high risk of bias in at least one domain; the clearest effects came from small subsets of trials (two to seven per outcome) and applied to older adults recovering from illness or injury, not to already-well community-dwellers.Meta-analysis · Age Ageing

    Exercise did not lengthen telomeres in women overall

    Across 17 studies of women, physical activity overall showed no significant relationship with telomere length, a marker of cellular ageing. A possible positive relationship appeared only for combined aerobic-plus-strength programmes and high-intensity interval training, and only in early and later adulthood rather than middle adulthood.

    Elastic bands build strength in frail older adults

    A systematic review of four randomised trials in 148 frail adults aged 65 or over found elastic-band resistance training over 12-28 weeks improved functionality, handgrip and lower-limb strength, especially in progressive, longer programmes; adding nutritional education further reduced frailty.

    Exercise lifted albumin; supplements added nothing

    In a pooled analysis of six randomised controlled trials in 330 older adults, exercise on its own significantly improved serum albumin levels, while combining exercise with nutritional supplementation showed no additional significant effect. Most included trials were judged to carry a significant risk of bias and overall certainty was rated low.

    Active later life tracks with longer, sharper years

    A coordinated analysis of 14 longitudinal studies covering 52,039 older adults found that more physical activity was associated with a lower risk of moving from unimpaired to mildly impaired cognition, a lower risk of death, and substantially longer estimated life expectancy. Evidence of benefit after impairment had already begun was present but less consistent between studies.

    What it shows: Pooled analysis of 14 observational cohorts (52,039 adults, mean baseline ages 69.9-81.73) from North America and Europe; physical activity was self-reported and not randomised, so this shows association, not proof that activity causes the slower decline.     Peak intensity tracked survival better than volume

    Across two accelerometry cohorts of adults with both diabetes and hypertension, 4,294 in UK Biobank and 530 in NHANES, higher peak 30-minute activity intensity was associated with lower all-cause mortality (hazard ratio 0.43 and 0.57 respectively), while total daily activity volume was associated with mortality in UK Biobank (HR 0.66) but attenuated to non-significance in NHANES (HR 0.95).

    What it shows: Observational analysis of two population cohorts with wrist accelerometry, 4,294 and 530 adults who had both diabetes and hypertension, with 731 and 123 deaths, so these are associations, not proof that harder activity extends life; reverse causation from existing illness and residual confounding cannot be excluded.Study · Acta Diabetol

    AR feedback boosted a gentle exercise's effect on frailty

    In a 12-week randomised trial of 117 older adults in long-term care, Baduanjin exercise guided by real-time augmented-reality feedback improved frailty more than the same exercise delivered by video or usual care, with extra gains in physical function, daily living ability, nutrition, cognition and mood.

    Exergames linked to fewer falls in older adults

    Pooling nine randomised trials in 1,385 adults aged 60 and over, exergaming was associated with a lower overall fall rate (incidence rate ratio 0.53) and fewer people falling at least once over 12 months versus usual care (risk ratio 0.75).

    Staying active after 60 tied to lower risk on many fronts

    Adults over 60 who stay physically active show lower risks of all-cause and cardiovascular mortality, breast and prostate cancer, fractures, recurrent falls, disability, cognitive decline, dementia and depression than inactive peers.

    Smoking beat lifespan genes as a mortality predictor

    In a Finnish twin cohort followed for a mean 17.5 years, a genetic score for longer lifespan was associated with modestly lower all-cause mortality, largely independent of lifestyle. Smoking 20 or more cigarettes a day and female sex were both stronger predictors than the genetic score.

    Having offspring linked to less frailty, not more

    In 95 geriatric female Rottweilers, dogs that had produced litters were 3 times more likely to be in the most robust fifth of the group in old age than dogs that never bred, and those with the most litters were the most robust. The predicted "frailty cost of reproduction" did not show up at all.

    Basic risk factors beat epigenetic clocks

    In a Finnish cohort of 1108 adults aged 34 to 49 followed for seven to nine years, a plain model of age, sex, smoking, alcohol, waist-hip ratio and BMI predicted new chronic disease better than any model that included a DNA methylation epigenetic clock. The authors argue clocks must demonstrate added value over cheap, familiar risk factors before being used clinically or sold as personal health monitoring.

    What it shows: Single population cohort of 1108 middle-aged Finns aged 34 to 49 at baseline with 7-to-9-year follow-up; results concern predicting incident non-communicable disease in relatively young adults and may not extend to older cohorts, longer follow-up or mortality outcomes.Study · Aging Cell

    Creatine's evidence now covers postmenopausal women.

    A 2026 meta-analysis in postmenopausal women found that creatine (around 5 g a day) alongside resistance training produced small but real gains in lean mass and leg strength, with no change in bone density and no safety signals.

    What it shows: Pooled from seven trials (608 women, median 38 weeks).
